Waiver of Liability.
The student stated above and his/her parents/guardians agree that Young Guns Elite Hockey shall not be held responsible for any injury, loss or damage whatsoever occurring to the participant while he/she is in attendance at Young Guns Elite Hockey.
I/We consent to his/her or my attendance on the terms of this waiver of liability or in the case of a guardian of a person under 19 years of age, grant our consent to the terms of this waiver.
I/we acknowledge reading this declaration and understand the conditions contained herein and agree to abide by all terms.
Permission.
I, the undersigned, hereby grant permission to Young Guns Elite Hockey (“Young Guns”) and its representatives to photograph and video me and/or my child (The Player), and otherwise capture my/their image, understanding that said images may be used royalty free in any current and future forms of media including online and social.
