Waiver of Liability.
The player/participant stated above and his/her parents/guardians agree that Young Guns Elite Hockey shall not be held responsible for any injury, loss or damage whatsoever occurring to the participant while he/she is in attendance at Young Guns Elite Hockey events.
Permission.
I, the undersigned, hereby grant permission to Young Guns Elite Hockey (“Young Guns”) and its representatives to photograph and video me and/or my child (The Player), and otherwise capture my/their image, understanding that said images may be used royalty free in any current and future forms of media including online and social.
I/we acknowledge reading this declaration and understand the conditions contained herein and agree to abide by all terms.
